Naval Hospital Bremerton to hold back-to-school/sports physicals Aug 14 and 28

BREMERTON — Naval Hospital Bremerton’s Family Medicine Department will again help families prepare for the upcoming school year and sport season by offering after-hours back-to-school/sports physicals from 5 to 6:45 p.m. on Wednesday, Aug. 14 and Wednesday, Aug. 28, on the second floor of NHB’s Family Medicine clinic.

There will be family medicine and pediatric providers, along with hospital corpsmen and nursing support staff standing by to assist parents and students to get this vitally important requirement crossed off on the back to school, “To Do List.”

In order to make the visit go smoothly students and parents are asked to come prepared. It is strongly encouraged for parents/students to complete all school/sports physical forms

prior to the appointment as this task is tedious and time consuming.

“Patients and parents need to bring the specific form - different school districts or sports organizations have different forms - for the student and to complete the demographic information ahead of time, such as address, date of birth, etc.,” said Charlene Johnson, Family Medicine business manager, also mentioning that students should

wear comfortable clothing for the exam.

“If all they need is to check to make sure immunization is up to date, (NHB) Immunization Clinic is open (8 a.m. to 4 p.m.) and they can walk in to have this done. We recommend this now rather than late August, as the volume of visits increase significantly towards end of August,”  Johnson added.

Other required items to bring include ID card which is required for students over age 10, TRICARE card, Immunization Record/shot card, and Medication Request forms. All youth or teen under the age of 18 must be accompanied by a parent or guardian or they will not be seen.

The visit will include the following: visual exam, vital signs such as blood pressure, pulse, respiration, height and weight, and the physical examination itself.

In order to qualify for the appointment, students must be TRICARE Prime beneficiaries enrolled in Family Medicine or Pediatrics at NHB. Patients are strongly encouraged to contact TRICARE Regional Appointment Center (TRAC) at 1-800-404-4506 to make an appointment.
